- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- Current state license as a Registered Nurse - Required
- CHPN certification - Preferred
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2 years Home health experience - Required.
Education
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelors of Science in Nursing - Preferred
Qualification Assessment
Must Have
Healthcare/ Homecare / Homebase - 1 year
OASIS (Outcome and Assessment Information Set) -1 year
Shift: M-F until shift is complete
On call info: On call required if requested
Travel Area: 50 mile radius around Roanoke VA
Guaranteed Hours: 32 hours
Productivity Units: 30 units
Qualifications: Home Health (not home-care) experience, familiar with OASIS documentation, Home-care Home-base tablet use preferred
This position is 40 hours a week Monday-Friday with a 30 unit per week productivity expectation. The productivity requires in home/person visits and timely documentation

Pay: $50.00 - $52.00 per hour
